1. Are the work-at-height activities identified? (Include routine and non-routine activities)

2. Are the employees/contractors/members of the public at risk from work at height identified?

3. How do your employees work at heights? Constantly > 4 hours per day? Daily? Weekly? Monthly, or every few months? Rarely, once or twice per year?

1. During work at height activities are the following hazards controlled: Employees falling from heights? Employees falling through a fragile roof? Objects falling on employees/members of the public? Slipping/collapsing access equipment? Slips/trips on elevated access ways?

1. Is working at heights avoided where possible?

2. Is work at heights properly planned and organized?

3. Has the work area been surveyed for hazards, eg. overhead power lines, unsuitable ground conditions, and moving machinery parts?

4. During work at height is there always a competent person present who can suspend work if conditions change, eg. weather, traffic, or scope of work?

1. Are fragile roofs identified, especially where fragility is disguised?

2. Are warning signs fixed on the approach to fragile roofs?

3. Is unauthorized access to roofs/roof spaces prevented?

1. Are ladders only used for light work of short duration?

2. Are portable ladders inclined at 75o (ratio 1:4) and secured top and bottom?

3. Are work platforms capable of supporting workers, equipment, materials, and the safe working loads indicated and inspected every six months?

4. Are work platforms with edge protection, such as tower scaffolds, cherry pickers, and scaffolding always used?

5. Are work platforms sufficiently wide to walk on without the risk of stumbling or losing balance?

6. Are properly constructed safety platforms always fitted if forklift trucks are used to gain access at heights?

7. Are portable ladders, steps, and all other access equipment regularly inspected?

8. Have workers been trained not to over-reach on ladders or work platforms?

9. Is a written log maintained and is access equipment removed from service if broken/damaged?

10. Is routine monitoring carried out to ensure that makeshift ladders or other makeshift access equipment is not being used?

1. Are open vertical drops identified and properly guarded when open?

2. Are handrails present on the open side of staircases?

3. Are open edges eg. on mezzanine levels provided with appropriate guard rails? (the use of chains, ropes, etc. is not suitable)

4. Are safe working methods used during the delivery removal of goods to mezzanine areas?

5. Are measures always taken to prevent falling objects from injuring others as a result of working at heights?

6. Is fixed edge protection provided to prevent falls where working at heights cannot be avoided?

1. Is storage properly planned which discourages climbing racking or pallets to access goods?

2. Are adequate provisions in place to allow safe access to stored/displayed goods?

3. Are staff trained in safe working methods to access high-level racking?

1. Have all workers who work at heights been assessed as competent or closely supervised by someone who is competent?

1. Do contractors provide safe written working procedures for work at heights, are they screened/instructed, and is their work monitored by a competent person?
