General Considerations

1. Is the equipment damaged?

Rigging Equipment

1. Is the structure able to support the loads imposed by the rigging equipment?

2. Is the rigging equipment properly designed for the application and has it been correctly assembled and installed as per the manufacturer’s instructions?

3. Are additional safety precautions taken when welding from a suspended scaffold?

4. Is all temporary rigging equipment properly tied back to a structurally sound anchorage?

5. Are davits, sockets, or other permanent equipment erected and installed correctly, and are inspections current?

6. When using counterweighted outrigger beams, does the number of counterweights match the outreach and hoist-rated load capacity per the counterweight formula?

7. Do test anchorage supports used in lieu of counterweights support all applied loads?

8. Are all welds in good condition?

9. Are all components undamaged and in good working condition, with mounting holes free from deformation and/or cracks?

Suspended Platform

1. Is the platform properly designed for the application and has it been correctly assembled per the manufacturer’s instructions or design drawings?

2. Are the platform stirrups in line with the rigging equipment?

3. Does the live load not exceed the platform-rated load capacity?

4. Are all visible welds in good condition and not visibly cracked, torn, or excessively corroded?

5. Are all components in good order, with no missing, bent, or damaged parts?

6. Are all mounting holes free from deformation?

7. Is the hardware grade 5 or better and in good condition, with lock nuts not worn?

8. Are the stirrup sheaves, bearings, and inlet wire rope guides in good condition?

9. Is the wire rope properly reeved through the sheaves?

10. Is the deck free from excessive debris that could cause a slipping hazard or affect the load capacity and/or proper functioning/installation of the component(s)?

11. Is the platform free from chemical corrosion and excessive abrasion damage?

12. When working near live power lines, is a safe distance maintained, with a minimum clearance of 10 feet required between scaffold equipment and power lines, and has the local power company been contacted regarding hazards?

13. Are weather conditions acceptable for the safe use of the platform, with wind speeds not exceeding 25 mph for a multi-point suspended platform or 20 mph for a single-point work cage or bosun’s chair?

Hoists, Wire Rope, And Power

1. Are the hoists properly selected for the application and have they been correctly installed as per the manufacturer’s instructions?

2. Have the manufacturer’s operating instructions been read, and understood, and are they available for reference?

3. Have the daily tests been performed according to the hoist operating manual?

4. Are the hoists in proper working order?

5. Are the electric cable and/or air hose connections secured, inspected, and safe for use?

6. Is the power supply adequate for the number and type of hoists being used?

7. Has the wire rope been inspected and is it in good working condition?

8. Is the length of the wire rope appropriate for the specific job and is it properly attached to the support equipment, with all fittings checked under load?

9. Are strain reliefs present and connected?

10. Do the emergency stop and secondary brakes operate correctly?

Fall Protection System

1. Do workers use proper fall arrest equipment at all times?

2. Has each vertical lifeline been inspected for safe use and is it correctly installed with edge protection to an independent anchor point capable of holding a 5,000-pound ultimate load?

3. Is each rope grab correctly installed and operating properly, with the size compatible with the lifeline?

4. Is each full-body harness properly inspected and adjusted?

5. Is each shock-absorbing lanyard inspected and installed correctly for safe use?
