1. Does the equipment have any obvious physical damage?

2. Do the operating controls work properly?

3. Do the emergency controls work properly?

4. Are the upper drive control interlocks functional(pedal, spring locks, etc)?

5. Is the emergency lowering functionality working?

6. Do the lower operating controls successfully override upper controls?

7. Is the control panel clean and protected from accidents?

8. Are all switches and mechanical guards properly installed?

9. Are motional alarms functional?

10. Are safety lights functional?

11. Are safety decals visible and clean?

12. Are all guard rails functional and in place?

13. Is the work platform extension clean and operational?

14. Is the lift free from cracks, damage, and leaks?

15. Are the tires in good condition?

16. Are breaks performing well?
