1. Are pay stations clearly marked and well lit?

2. Are pay stations in good working order and accept all designated methods of payment?

3. Is the pay station regularly serviced?

1. Are staff in appropriate uniform?

2. Are staff adhering to the equipment maintenance schedule?

3. Are staff keeping equipment maintenance reports and records up to date?

4. Are fire and other emergency numbers displayed in the staff area?

5. Is a stocked first-aid kit accessible?

6. Are all required insurance and safety certificates displayed in the staff area?

7. Are staff office and restrooms clean and free from clutter?

8. Are staff trained to record and report all accidents?

9. Do staff follow the designated cash collection and deposit procedures?
