Utilities

1. Have utility locates been contacted at least 2 working days prior to the start of digging, and have they been appropriately marked and identified?

2. Have underground installations been protected, supported, or removed when the excavation is open?

3. Have surface encumbrances such as utilities, utility poles, foundations, transformer vaults, or other structures been supported or removed?

4. Is the equipment operator aware of energized overhead uninsulated powerlines and the required approach distances (10' minimum for lines <50kV; 20' minimum for unknown voltage; with a dedicated spotter)?

Excavation / Trench

1. Has the soil been classified using 1 visual and 1 manual method, or default Type C?

2. Are employees protected from loose rocks or soil?

4. Have spoils, materials, and equipment been set back at least 2' from the edge of the excavation?

1. Where a hazardous atmosphere could reasonably exist, is the atmosphere being tested for low or high oxygen, hazardous vapors, and toxic gases before employees enter the excavation?
