MAR/EMAR Audit

A checklist to document and update the Medication Administration Record (MAR) or electronic Medication Administration Record (eMAR) while administering medications and changing prescriptions. This checklist can also be used to evidence medication audits as part of internal/external reviews.

MAR/EMAR Check

1. Staff updating the MAR/EMAR has seen the original prescription.

2. All MAR/EMAR charts clearly identify the start date including year.

3. All entries show the name, strength and form of medicine and full directions for use.

4. All entries show any additional information/warnings (take with of after food).

5. MAR/EMAR charts signed for each individual medicine administered including "bought" medications.

6. Appropriate code recorded on the chart for any medication that hasn't been given and it's cross-referenced to an entry in the care plan.

7. All medication is available for staff to administer at the time of their visit (medication should not be out of stock).

8. Any variable dose that is prescribed, the administered dosage is carefully recorded.

9. Any prescribed warfarin the administration dosage is clearly stated on the MAR/EMAR chart and can be cross-referenced to information provided by the GP practice.
