Use this ICU Rounds Checklist for a systematic approach to patient care during ICU rounds, ensuring comprehensive assessment and healthcare for patients.

1. Is sedation reduced?

2. Is analgesia reduced?

3. Is delirium assessed, addressed, and prevented?

4. Is the patient a candidate for a Spontaneous Awakening Trial (SAT) and/or Spontaneous Breathing Trial (SBT)?

5. Is the total fluid balance goal reviewed and is the targeting 12h or 24h?

6. Is the patient at goals for lung protective ventilation?

7. Is the head of the bed elevated to 30 degrees?

8. Is a skin breakdown assessment for prevention completed?

9. Is enteral nutrition a goal?

10. Is blood glucose at goal?

11. Is deep venous thrombosis prophylaxis in place?

12. Is gastric ulcer prophylaxis initiated under the following criteria: Coagulopathy (INR>1.5, PTT>2x normal, Plt < 50k)

13. Is gastric ulcer prophylaxis initiated under the following criteria: Mechanical ventilation > 48h

14. Is gastric ulcer prophylaxis initiated under the following criteria: History of GI bleed/ulcer within the past year

15. Is gastric ulcer prophylaxis initiated under the following criteria: TBI, SCI, or burn

16. Is gastric ulcer prophylaxis initiated under the following criteria: or >2 minor risk factors (sepsis, ICU stay >1 week, occult GI bleed > 6 days, glucocorticoid therapy, NSAID use, antiplatelet use)?

17. Are antibiotics narrowed or discontinued?

18. Is the patient a candidate for early mobility, and is physical therapy consulted?

19. Is a Foley catheter needed?

20. Is a central venous/arterial catheter needed?

21. Is the family updated on the patient's condition?

22. Are the patient's goals of care and code status current?
