1. Has the worker read and understood the company Health and Safety Policy and Procedures document prior to the induction?

2. Is the location of workplace amenities such as toilets, wash areas, and lunch rooms shown to the worker?

3. Are workplace emergency procedures discussed, including the location of fire extinguishers, first aid kits, exits, assembly areas, etc.?

4. Are significant workplace hazards reviewed with the worker using Hazard Cards?

5. Using the Hazard Cards, are any equipment/appliances posing potential risks explained or shown to the worker (e.g., photocopier, kitchen appliances, cutting equipment)?

6. Is the process for reporting and recording accidents and near-miss incidents explained?

7. Are the safety representatives and information about safety meetings provided?

8. Are procedures for workplace visitors explained?

9. Are any areas that workers cannot enter explained or shown?

10. Are specific dangerous machinery identified, with an explanation that workers are not to touch or operate them unless they are fully trained or supervised by someone qualified?

11. Is the worker's driver's license viewed and copied, full and current?

12. Is a vehicle induction completed including an explanation of the Vehicle Checklist and when it needs to be completed?

13. Is the Vehicle Hazard Card looked through and the Company’s Vehicle Policy explained?
