1. Have all required legislated obligations been completed prior to installation?

2. Has all required licensing and testing been completed prior to or on installation?

3. Has all required specialist signage been installed?

4. Is baseline health surveillance required?

5. Is there an English language version of the instruction/operating manual available?

6. Have all modifications been completed and approved by relevant authorities?

7. Does the equipment NOT create emissions or any other by-product which may impact the environment?

8. Has a Safe Work Procedure (SWP) been completed for the equipment?

9. Are training, work practice, and supervision changes required?

10. Have all staff who require training in the use of the equipment completed it prior to commissioning?

11. Are staff training records completed and properly stored in the database?

12. Has the contractor company and individual workers completed the contractor induction?

13. Is a HAZOP required for this equipment? (Do it for high-risk/complex equipment)

14. Has the Pressure Vessel Compliance Checklist been completed?

15. Are Specific Emergency Plans and relevant procedures/equipment in place?

16. If required, are health surveillance and testing programs in place?

17. Are machine guarding and emergency stops in place and working as designed?

18. Have all electrical wiring and supply issues been resolved?

19. Have all gas storage and supply issues been resolved?

20. Have all radiation sources storage, handling, and disposal issues been identified and resolved?

21. Has a maintenance plan and contract for scheduled maintenance and breakdown been arranged?

2. Has a visual inspection been carried out to assess the physical damage of components and cases?

3. Are all items packed in transit removed from the combustion chamber?

4. Has the boiler been assembled as per the manufacturer's instructions, including casing and baffles where appropriate?

5. Has the boiler been pressure tested and certificated?

6. Are boilers filled with water and isolation valves open?

7. Are the burner-to-boiler fixings correct as per the manufacturer's instructions?

8. Are water pumps working to provide sufficient circulation/heating load?

9. Is the burner flame tube insulation sealing correct at the appliance body as per the manufacturer's instructions?

10. Is there adequate clearance around the boiler and burner for servicing?

11. Is the boiler door aligned and sealed correctly?

12. Is flue installation safe, fitted correctly, and sealed to the appliance?

13. Does the permanent ventilation comply with current installation standards and regulations?

14. Are appliance electrical wiring checked for safety?

15. Is the wiring of the burner to the appliance correct and according to the manufacturer's instructions?

16. Are external controls and interlocks connected and fully operational (BMS, fan flue, flow switches, dampers, diverter valves, etc.)?

17. Are boiler thermostats fitted correctly to the manufacturer's instructions (on/off, high/low, high limit)?

18. Are modulating burners probes fitted and wired to manufacturers instructions?

19. Are all safety systems fitted and fully operational (e.g. pressure relief/safety valves etc.)?

20. Does the gas pipework safely fit and support with the correct means of isolation?

21. Is the gas pipework tested for tightness and purged (certificates and fuel available)?

22. Is the gas booster or non-appliance-specific gas valve safety interlocks connected?

23. Is the adequate access to the gas meter?

24. Are oil supply pipe(s) correctly fitted, purged, and supported (check inline filter has been fitted)?

25. Is the oil pump set for the single or two-pipe system as applicable (fuel available)?

26. Is the rotation of 3-phase motors relative to the burner/booster checked?
