Justification & Authorization

1. Every radiograph taken must have a documented clinical reason (justification) recorded in the patient’s clinical notes.

2. Each radiograph must be authorized by a qualified practitioner before the exposure occurs.

3. The patient’s pregnancy status must be checked (where applicable) and the result recorded in the notes.

4. Evidence must exist that previous radiographs were reviewed to avoid unnecessary exposure before deciding to take new ones.

Image Quality & Grading

1. At least 95% of the audited radiographs must be graded as "Level 1," representing excellent quality with no errors.

2. No more than 5% of the audited radiographs should be graded as "Level 2," representing diagnostically acceptable images with minor errors.

3. The audited sample must contain zero "Level 3" (unacceptable/non-diagnostic) images, unless they have been formally logged as "Repeats".

4. For any "Level 2" or "Level 3" images, the specific fault—such as positioning, coning, or processing errors—must be identified and recorded.

Clinical Evaluation & Reporting

1. A formal written report of the findings for every radiograph must be recorded in the patient’s clinical notes.

2. The clinical report must cover all visible areas on the X-ray, including any incidental findings.

3. The "Quality Grade" of the image must be recorded alongside the clinical report.

4. The names of the individual who took the X-ray and the individual who interpreted it must be clearly documented.

Equipment & Radiation Protection File

1. The Radiation Protection File (RPF) must be current and include the local rules and the name of the Radiation Protection Supervisor (RPS).

2. All X-ray units must undergo a formal "Critical Examination" and "Functional Test" within the required statutory timeframe.

3. Rectangular collimation must be used on all intra-oral units to minimize the radiation dose to the patient.

4. Exposure settings (kV, mA, and time) for various tooth types and patient sizes must be clearly displayed near the X-ray unit.

5. All staff members involved in radiography must maintain up-to-date, verified Continuing Professional Development (CPD) in radiation protection.

Quality Assurance (QA) & Safety Protocols

1. A "Reject Analysis" log must be maintained to track and address recurring faults in X-ray images.

2. Digital sensors or phosphor plates must be checked for physical damage or artifacts on a regular basis.
