1. What is the truck's ID, make, model, and license plate number?

2. What is the current mileage on the odometer?

3. What is the scheduled route or delivery zone for the day?

4. What is the inspector's name and date/time of the inspection?

1. Are all sides of the vehicle free from new damage, including scratches, dents, or signage wear?

2. Are headlights, taillights, indicators, and hazard lights functioning and clean?

3. Are the mirrors clean, properly adjusted, and not cracked or loose?

4. Is the company branding visible and undamaged?

1. Are all tires properly inflated and free from punctures or irregular wear?

2. Is there sufficient tread depth on all tires?
