Utilize this Central Line Bundle Checklist to approach central line care, ensuring infection prevention and patient safety during invasive procedures.

1. Is informed consent obtained?

2. Is supervision obtained if needed (as per designated roles)?

3. Did the team perform a time-out/briefing?

4. Did the team confirm hand washing/sanitizing immediately prior?

5. Did the operator(s) wear a cap, mask, sterile gown/gloves, and eye protection?

6. Did the supervisor wear a cap, mask, sterile gown/gloves, and eye protection?

7. Did the assistant wear a cap, mask, isolation gown, and gloves, with eye protection if at risk for entering the sterile field? (If entering a sterile field, use a sterile gown and gloves.)

8. Is the patient properly positioned to prevent air embolism : For Chest/EJ: Trendelenburg (Head of Bed < 0 degrees)

9. Is the patient properly positioned to prevent air embolism : For Femoral: Supine

10. Is the procedure site sterilized with chlorhexidine?

11. Is the site allowed to dry?

12. Is the sterile technique used to drape from head to toe?

13. Is local anesthetic and/or sedation utilized?

1. Is a sterile field maintained?

2. Are lumens not cut during the procedure?

3. Are any ports not used during insertion clamped (to avoid air embolism, clamp all but the distal port)?

4. Is a qualified second operator obtained after three unsuccessful sticks (except if emergent)?

5. Is blood aspirated from each lumen (to avoid air embolism and ensure intravascular placement)?

6. Is central venous pressure (CVP) transduced or estimated by fluid column (to avoid arterial placement)?

1. Is blood cleaned from the site using an antiseptic agent (chlorhexidine), and a sterile dressing applied?

2. Is placement verified by X-ray (ensuring the tip is in the superior vena cava/right atrium junction)?
