1. What time is the antibiotic administered?minutes before incision.

3. What is the antibiotic dosage appropriate for the patient's weight?

4. Is the surgical hand scrub performed?

5. Is appropriate Personal Protective Equipment (PPE) worn? (Including scrub gown, scrub suit, cap, sterile gloves, eye protection, and surgical mask)

6. Is the patient covered with a drape?

7. Did the drape separate surgical instruments, equipment, and supplies?

8. Is proper ventilation maintained throughout the surgery through the following: maintaining positive pressure

9. Is proper ventilation maintained throughout the surgery through the following: a minimum of 15 air exchanges per hour with 3 being fresh air
