1. Is the cylinder in an acceptable shape?

2. Is the current hydro test date valid?

3. Does the cylinder valve function properly?

1. Are the straps in acceptable condition?

2. Is the backpack in acceptable condition?

3. Are the hoses in good shape?

1. Are the straps and buckles moving freely?

2. Are all the straps working and is the mask in good condition?

3. Is the nose cup seated properly?

4. Does the regulator connect/disconnect with the face mask easily?

5. Is the regulator cover in good condition?

6. Is the emergency bypass valve working correctly and freely?

7. Is the low-pressure alarm working correctly?

8. Is the PASS device working correctly?

9. Is the PASS device activated on its own to ensure that it is functioning correctly with the escalated alarms?

1. Are there any additional deficiencies?

1. Is this form filled out by the fire chief or their designate?

2. Have all the deficiencies been repaired for all items noted above?
