Use the Radiation Safety Checklist to assess working environments through inspections of radioisotope safety, electrophoresis, radiation devices, etc.

1. Is the “Notice to Employees” poster displayed in appropriate numbers and locations?

2. Are appropriate radiation warning signs posted at the entrance(s)?

3. Are emergency procedures and phone numbers posted?

4. Are radioactive material and radiation-producing devices secured to prevent unauthorized access?

5. Is the “State Regulations for Protection Against Radiation” policy available?

6. Is the “Radiation Safety Manual” available?

7. Do all personnel have documented training of the appropriate type?

8. Are the appropriate personnel supplied with dosimeters?

1. Is the work area orderly and free of clutter?

2. Is food, drink, applying cosmetics, etc., prohibited?

3. Is mouth pipetting prohibited?

4. Is personal protective equipment available and worn appropriately?

5. Are hands washed before exiting the work area?

6. Are accurate and up-to-date isotope receiving logs kept and available for inspection?

7. Are accurate and up-to-date isotope use logs kept and available for inspection?

8. Are semi-annual inventories performed, documented, and copies sent to RSO?

9. Is the area surveyed for contamination at least weekly?

10. Is a wipe test and contamination survey log kept and available for inspection?

11. Have survey meters been calibrated within the last twelve (12) months?

12. Are survey meters in good working order?

13. Are survey meter check sources available and used?

14. Are bioassays performed, and documented, and copies sent to RSO as necessary?

15. Are refrigerators, hoods, storage cabinets, and waste receptacles clearly and appropriately labeled?

16. Are isotopes marked with name, activity, and assay date?

17. Is waste properly labeled and stored before disposal?

18. Are accurate and up-to-date isotope disposal logs kept and available for inspection?

19. Are radioactive markings obliterated on non-contaminated items before disposal via the building’s solid waste?

20. Are semi-annual leak tests performed on sealed sources and records maintained?

1. Are written operating procedures readily available?

2. Is the power supply marked “DANGER – HIGH VOLTAGE?”

3. Is the sequencing unit marked “DANGER – HIGH VOLTAGE?”

4. Is the power “ON” lamp on the power supply working and visible from the work area?

5. Is the power “ON” switch readily accessible?

6. Are all electrical contacts guarded against operator contact?

7. Are all power leads insulated and undamaged?

8. Is the AC power cord a grounded type and undamaged?

9. Is the unit cover safety interlocked?

10. Are the unit and power supply located away from the sink?

11. Are appropriate personnel trained in CPR?

3. Are quarterly safety device tests performed and documented in a log?
