This inspection checklist can be used by Assisted Living Facilities and Care Homes to ensure that COVID-19 safety precautions are followed by care workers and staff (as per Occupational Safety and Health Administration guidelines).

1. Sick workers instructed to stay home?

2. Workers and residents regularly screened for signs and symptoms consistent with COVID-19?

3. Workers and residents quarantined if they have been exposed to an individual with COVID-19?

4. CDC guidance on resident visitation policies being followed?

5. Visitors asked to inform the facility if they develop a fever or symptoms consistent with COVID-19 within 14 days of their visit?

6. Physical distancing practices implemented to maintain at least six feet between workers / residents / visitors, including when workers perform their duties and during breaks?

7. Are break periods staggered to avoid crowding in breakrooms?

8. Are alternatives to large in-person gatherings (e.g., staff meetings, resident activities) being used?

9. Has a qualified ventilation professional evaluated the building’s HVAC system?

10. Handwashing facilities and alcohol-based hand sanitizer with at least 60 percent ethanol or 70% isopropanol provided throughout the facility?

11. Frequently-touched surfaces in resident rooms, staff work stations, and common areas cleaned and disinfected regularly?

12. Hospital-grade cleaning chemicals being used?

13. Workers have and use any personal protective equipment (PPE) they need to perform their jobs safely?

14. The facility’s stock of PPE, burn rate, and supply chains being continuously monitored?

15. Process for cleaning and decontaminating PPE, such as face shields and goggles, in place?

16. Workers trained on COVID-19 policies and procedures in a language they understand?

17. Do the policies encourage workers to report any safety and health concerns?
