1. Have areas in the workplace where noise levels exceed 85 decibels been identified and evaluated?

4. Is there an ongoing preventive health program in place to educate workers about safe levels of noise, exposures, effects of noise on their health, and the use of personal protection?

5. When workers are subjected to sound exceeding the levels in 29 CFR 1910.95, Table G-16, are engineering controls, administrative control, and/or personal protective equipment used to reduce the level of sound exposure to be within the levels of the table?

6. Is approved hearing protective equipment (noise attenuating devices) available to every worker working in noisy areas?

7. Are workers properly fitted and instructed in the use of hearing protectors?
