Use this template to audit night shifts in the healthcare setting, examining security, staff, medication, health & safety and wellbeing of residents during the night.

Medication

1. All prescribed medication has been given appropriately and signed for on the MAR/EMAR.

4. All cross corridor doors closed during the night shift as per fire policy.

1. All external doors should be locked as required with key code only.

2. Front door code to be the company standard code which is changed periodically.

3. Car park lighting should be appropriate to prevent falls with all external lights in working order.

Team Members

1. Rota checked to ensure numbers are appropriate and that team members are on the allocated unit.

2. All team members observed to ensure they are working appropriately.

3. All team members are wearing the appropriate uniform including name badges.

4. Completed handover to all team members prior to the commencement of the shift (check with team and the written handover).

5. Team members are aware of allocation for breaks and allocation of residents.

Residents

1. Minimum of 5 residents to be checked to ensure they have the call bells available in reach.

2. Residents have access to a drink if they wish (water bottle or juice at the bedside).

3. Check residents' fluid balance charts and ensure these have been filled out appropriately.

4. Check residents' repositioning charts to ensure they are completed appropriately.

5. Light snacks available for the residents if they are hungry throughout the night.

6. Daily records completed appropriately and at the right time.

7. Residents who are awake are engaging in meaningful activity or conversation.
