This forklift pre-start inspection checklist is to be completed by operators every day prior to using the forklift, as part of routine workplace health and safety requirements. It examines the overall condition of the forklift and ensures it is safe to be used.

1. Any visual damage?

2. Any damage to signage or rating plates on forklift?

3. Any faults with the emergency stop?

4. Any faults with the control functions?

5. Any unusual noise or vibration?

6. Any damage to tines or load backrest?

7. Any damage to overhead guard or ROPS?

8. Any faults with tires including wheel nuts, inflation pressure and tread?

9. Any fluid leaks?

10. Any fluids outside of their appropriate range including engine, hydraulic, transmission, brake fluid, coolant and power steering?

11. Any faults with reverse beeper, lights or horn?

12. Check this box to indicate that you have included a picture and description of any faults identified.

13. Please sign to indicate that you have completed the prestart check and that you will tag the vehicle out and report it to VMS if not safe to operate.
