2. Are spots on the rubber flooring cleaned?

3. Is the floor mopped?

4. Is the floor allowed to dry completely?

5. Is the floor disinfected?

6. Daily: Are the floors cleaned and disinfected?

7. Periodic: Is burnishing performed periodically?

8. Yearly: Is the floor recoated once a year?

1. Is the facility properly cleaned?

2. Are high-touch surfaces disinfected regularly?

3. Are areas and equipment sanitized according to guidelines?

4. Is staff training provided protocols on cleaning, disinfecting, and sanitizing?

5. Are cleaning and safety procedures clearly communicated to fitness members?

6. Are the hands washed for 20 seconds before starting?

7. Is the appropriate Personal Protective Equipment (PPE) worn?
