2. Is the gown inspected for defects before use?

3. Does the gown fully cover the torso from neck to knees and arms to wrists?

4. Is the gown fastened securely at the back of the neck and waist?

5. Is the mask or respirator positioned correctly over the nose and mouth?

6. Is the mask or respirator secured with ties or elastic bands around the head?

7. Is the nosepiece of the mask or respirator fitted snugly to the nose bridge?

8. Is a proper seal check performed if using a respirator?

9. Are goggles or a face shield placed correctly over the face and eyes?

10. Are goggles or a face shield adjusted for a secure and comfortable fit?

11. Are gloves inspected for damage before wearing?

12. Are gloves properly worn, covering the wrists, and overlapping with the gown cuffs?

13. Are gloves replaced if torn or contaminated during the process?
