Use this Dementia Audit Checklist to evaluate care settings for dementia suitability, assessing signage, lighting, etc. to ensure a safe and comfortable environment. This audit can be conducted quarterly. 1. Do you have a quiet space for someone who might be feeling anxious or confused?

2. Does the quiet space offer a few minutes of calm with a supportive person when needed?

1. Are your signs clear, with bold face and good contrast between text and background?

2. Is there a contrast between the sign and the surface it is mounted on?

3. Are signs fixed to the doors they refer to, rather than on adjacent surfaces?

4. Are signs placed at eye level and well-lit?

5. Are signs clear without use of abstract images

6. Are signs placed at key decision points for navigation?

7. Are signs for toilets and exits clear and easy to understand?

8. Are glass doors clearly marked?

1. Are entrances well-lit and make use of natural light where possible?

2. Pools of bright light or shadows should be avoided, please confirm none are evident

1. Are floor surfaces non reflective and non-slip

2. Are carpets plain colours and patterns

3. Are changes in floor finish flush rather than stepped? (Avoid steps to reduce trip hazards.)

1. Do you have a changing room where an opposite sex carer or partner can assist? If not, are staff briefed to meet this need sensitively?

2. Do you have a unisex toilet or facility for privacy and assistance?

3. Are toilet seats a contrasting colour to the walls and rest of the toilet? (This aids visibility.)

1. In larger premises, do you provide seating areas where people might wait?

2. Does seating look like traditional seating? (Avoid abstract designs; simpler, conventional seating is preferred.)

1. Have you considered using landmarks (e.g., paintings, plants) to assist with navigation? (Landmarks should be attractive and interesting.)

1. Does the environment support individuals with dementia effectively and compassionately?
