CQC Self-Assessment - Well Led

Use the CQC Self Assessment Checklist to evaluate and improve the quality of care, in line with the Single Assessment Framework's "Well-Led" key question and quality statements. It can be conducted bi-annually, or more frequently if issues are uncovered.

1. There is underpinning evidence which supports the ethos of a shared vision, strategy and culture.

2. The service is able to demonstrate that this underpinning evidence is based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities.

1. There is evidence that the service has in place inclusive leaders at all levels who understand the context in which care and support and treatment is delivered.

2. These inclusive leaders are able to demonstrate that they embody the culture and values of their workforce and organisation and that they have the skills, knowledge, experience and credibility to lead effectively.

3. Leaders can demonstrate that they undertake their roles with integrity, openness and honesty.

1. There is evidence within the service of a positive culture where people (residents and staff) feel that they can speak up and that their voice will be heard.

2. The service can evidence that it works towards making a continuous positive contribution in reducing this negative impact and how it supports people (staff, residents & visitors) to do the same.
