1. Has a thorough risk assessment been conducted to identify potential hazards within the confined space?

2. Is there a written Confined Space Entry Program in place?

3. Are confined spaces properly labeled and secured to prevent unauthorized entry?

4. Have all personnel received documented training specific to confined space entry and the use of required PPE?

5. Is the confined space entry permit completed and authorized?

1. Is the appropriate respiratory protection selected based on identified atmospheric hazards (e.g., oxygen deficiency, toxic gases)?

2. Are respirators inspected for cleanliness, proper function, and integrity?

3. Have respirators been fit-tested for each user?

4. Are replacement filters or cartridges available and within their expiration dates?

1. Are hard hats free from cracks, dents, or other damage?

2. Is the suspension system inside the hard hat intact and properly adjusted?

3. Are safety glasses or goggles free from scratches, cracks, or other damage?

4. If face shields are required, are they clean and free from damage?

5. Is hearing protection available and appropriate for the noise levels expected in the confined space?

6. Are earplugs or earmuffs clean and in good condition?

1. Is protective clothing appropriate for the identified hazards (e.g., chemical-resistant suits, flame-resistant clothing)?

2. Is the clothing free from rips, tears, or other damage?

3. Is the clothing properly fitted to the wearer?

4. Are gloves selected based on the specific hazards (e.g., chemical exposure, abrasion, thermal hazards)?

5. Are gloves free from cuts, tears, punctures, or other damage?

6. Are safety boots or shoes appropriate for the hazards (e.g., slip-resistant, steel-toed, chemical-resistant)?

7. Is footwear free from damage and excessive wear?

1. If there is a risk of falling, is appropriate fall protection equipment (e.g., harnesses, lifelines) available and in good condition?

2. Has the fall protection equipment been inspected and maintained according to the manufacturer's guidelines?
