The Chemical Risk Assessment form is an important tool to identify chemical hazards and appropriate control measures, as part of workplace health & safety inspections aimed at decreasing risk to workers and visitors.

1. Name of the chemical

2. What is the chemical used for?

3. Location of the process or work where the chemical is used

4. What state or form is the chemical in? (Gas, Vapor, fume, liquid, mist, solid, etc.)

5. What is the hazard posed by the chemical? (Acute toxicity, very toxic, corrosive, explosive, oxidising, flammable, etc.)

6. Which route(s) of exposure does the substance take? (Eyes, skin, inhalation, ingestion, etc.)

7. What are the risks to health from exposure to the substance?

1. What are the containment measures?

2. What are the disposal methods?

1. What are the hazard prevention practices being used currently?

2. Are the current hazard prevention practices enough or are new practices recommended?

1. Is the exposure sufficiently controlled?

2. What is risk rating after following control measures?
